The options are:

Personalized Name and Number on the back of your item for $10. For this option please specify the name and number that you would like in the notes form after you choose this service.

Name/Number/Glitter $20 - This option is to have all the options added on for your particular item. Please specify the name and number that you would like in the notes form after you choose this service.

This is per item, so for example if you have 2 items that you need customized, you will have to add two of the custom items to your cart.
